Camden Homelessness System - Blocks and gaps reporting form

Overview

As part of Camden’s Homelessness System Transformation, we want to capture the challenges and barriers encountered by people experiencing homelessness and other disadvantages, and the services that support them. 

Each time you hold a 'Team Around Me' or a Multi-disciplinary Team Meeting for a client who is encountering an issue which remains unresolved, please fill out this brief anonymised form. The form asks for some basic demographic information, and then you can choose from a list of system blockages, or add your own if it’s not represented. 

Please do not share any identifiable data related to clients, including their name and date of birth. 

Each form should take less than 5 minutes. 

Why your views matter

We will use this anonymised data to report back to senior leaders in Camden, to highlight trends in system blockages and barriers, and the data will form evidence to support new initiatives which address unmet needs.
