Your experience of services provided by Camden and Islington NHS Foundation Trust

Overview

We would like to invite you to share your experiences of the mental health services provided by Camden and Islington NHS Foundation Trust. This survey has been requested by Councillors in the Health Scrutiny Committee at Camden council. Your views are really valuable and will help to develop local services.

It will take approximately 15 minutes to complete. Your responses will be completely anonymous and all information will be kept confidential and not shared with anyone outside of Public Health. After you complete the survey, you will have the option to provide your name and contact details to be entered in to a prize draw for one of three £20 gift vouchers or for participation in a follow up group discussion. The findings which will be shared afterwards will NOT have your name or any contact details on it.
